Your Initial Visit

We ask that you print out and complete the "New Patient" forms available on this website prior to arriving at the office. If you do not fill out the forms prior to your visit, we ask that you please arrive 30 minutes prior to your visit to complete necessary paperwork. Please plan on spending 1-2 hours in the office.

Office Policies

At every visit, please bring your photo ID, medical insurance information and referral from your primary care doctor if required by your insurance. In order to make your visit more efficient, we ask that you also bring copies of any pertinent medical records, an updated medication list, and your blood glucose meter, glucose log sheets and diet diary (if applicable).

If you need to cancel your scheduled appointment for any reason, please provide a 24 hour notice. Failure to do so may result in a fee.

Tri-County Endocrinology is a fragrance free office. Please refrain from using any scented products for the comfort of our staff and patients.

Insurance and Payment Information

Tri-County Endocrinology participates with most major insurers. We currently participate in the Blue Care Network HMO network and HAP. For Blue Care Network HMO, a referral is needed for all visits at the time of service. Please call to inquire whether Tri-County Endocrinology participates in your insurance plan.

Insurance co-pays, deductibles as well as any prior balances will be collected prior to your physician visit. If you do not have health insurance benefits or office visit consultation coverage, payment is expected in full at the time of service. For your convenience, we do accept personal check, cash, Visa, Mastercard or Discover.
